Novembre 2023: Le pembrolizumab (Keytruda, Merck) a été approuvé par la Food and Drug Administration (FDA) comme traitement néoadjuvant en association avec une chimiothérapie contenant du platine et comme traitement adjuvant post-chirurgical pour les tumeurs résécables du cancer du poumon non à petites cellules (CPNPC) mesurant 4 cm ou plus de diamètre, lorsqu'il est associé à une chimiothérapie contenant du platine.
KEYNOTE-671 (NCT03425643), a multicenter, randomized, double-blind, placebo-controlled trial involving 797 patients with AJCC 8th edition resectable Stage II, IIIA, or IIIB NSCLC who had not been previously treated, assessed the drug’s efficacy. Patients undergoing platinum-based chemotherapy were randomized (1:1) to receive pembrolizumab or a placebo every three weeks for four cycles (neoadjuvant treatment).
Subsequently, for a maximum of thirteen cycles (adjuvant treatment), patients were administered either continued single-agent pembrolizumab or a placebo every three weeks. The surgical window and chemotherapy specifics are available at the link to the drug label above.
Les principaux critères de jugement de l'efficacité étaient la survie sans événement (EFS) et la survie globale (SG) évaluées par l'investigateur. La SG médiane pour les patients recevant le placebo était de 52.4 mois (IC à 95 % : 45.7, NE) et n'a pas été atteinte dans le bras pembrolizumab (IC à 95 % : non estimable [NE], NE] ; valeur p = 0.0103). Le risque relatif [HR] était de 0.72 [IC à 95 % : 0.56, 0.93] ; valeur p = 0.0103]. La SSE médiane dans le bras placebo était de 17 mois (IC à 95 % : 14.3 ; 22.0) contre 17 mois dans le bras pembrolizumab (IC à 95 % : 34.1 mois, NE) (HR 0.58 [IC à 95 % : 0.46 ; 0.72] ; valeur p = 0.0001).
The adverse reactions most frequently reported by 20% or more of the patients in KEYNOTE-671 were as follows: nausea, fatigue, neutropenia, anaemia, constipation, decreased appetite, decreased white blood cell count, musculoskeletal pain, rash, congestion, vomiting, diarrhoea, and dyspnea.
A comparatively lower rate of adverse reactions prevented surgery for 6% of patients in the pembrolizumab arm who received neoadjuvant treatment, as opposed to 4.3% in the placebo arm. In addition, 3.1% of patients who received neoadjuvant treatment and surgery in the pembrolizumab arm experienced surgical delays as compared to 2.5% in the placebo arm. Le pembrolizumab est prescrit à la posologie de 200 mg toutes les 3 semaines ou de 400 mg toutes les 6 semaines. Lorsqu'il est administré le même jour qu'une chimiothérapie, le pembrolizumab doit être administré au préalable.
