Août 2022: For adult patients with unresectable or metastatic HER2-low (IHC 1+ or IHC 2+/ISH) breast cancer who have received prior chemotherapy in the metastatic setting or experienced a disease recurrence during or within six months of finishing adjuvant chemotherapy, the Food and Drug Administration has approved fam-trastuzumab deruxtecan-nxki (Enhertu, Daiichi Sankyo, Inc.).
DESTINY-Breast04 (NCT03734029), a randomised, multicenter, open-label clinical study that enrolled 557 patients with metastatic or unresectable HER2-low breast cancer, served as the foundation for the effectiveness analysis. In the study, there were two cohorts: 494 individuals with hormone receptor positivity (HR+) and 63 patients with hormone receptor negativity (HR-negative). In a centralised laboratory, IHC 1+ or IHC 2+/ISH- was used to characterise HER2-low expression. Enhertu 5.4 mg/kg was administered intravenously every three weeks to patients who were randomly assigned (2:1) to receive it or the physician’s choice of chemotherapy (N=184, including eribulin, capecitabine, gemcitabine, nab-paclitaxel, or paclitaxel).
The progression-free survival (PFS) rate in patients with HR+ cancer du sein, as determined by a blinded independent central review using RECIST 1.1, served as the key effectiveness measure. PFS in the total population (all randomised HR+ and HR-negative patients), overall survival (OS) in HR+ patients, and OS in the total population were secondary effectiveness endpoints.
L'âge des patients variait de 28 à 81 ans, 57 ans étant la médiane, tandis que 24 % avaient 65 ans ou plus. La liste suivante de données démographiques sélectionnées a été fournie : 99.6 % de la population est féminine, 48 % est blanche, 40 % est asiatique, 2 % est noire ou afro-américaine et 3.8 % est hispanique/latino.
La SSP médiane dans la cohorte HR+ était de 5.4 mois dans le groupe chimiothérapie et de 10.1 mois dans le groupe Enhertu (risque relatif [HR] 0.51 ; IC à 95 % : 0.40, 0.64 ; p0.0001). Dans le bras Enhertu, la SSP médiane était de 9.9 mois (IC à 95 % : 9.0, 11.3), alors que pour ceux qui prenaient une chimiothérapie, elle était de 5.1 mois (IC à 95 % : 4.2, 6.8) (HR 0.50 ; IC à 95 % : 0.40, 0.63 ; p0.0001).
Dans la cohorte HR+, la SG médiane pour les bras chimiothérapie et Enhertu, respectivement, était de 17.5 mois (IC à 95 % : 15.2, 22.4) et de 23.9 mois (IC à 95 % : 20.8, 24.8) (HR 0.64 ; IC à 95 % : 0.48 , 0.86 ; p = 0.0028). Dans la population générale, la SG médiane pour le groupe Enhertu était de 23.4 mois (IC à 95 % : 20.0, 24.8) et pour le groupe chimiothérapie, elle était de 16.8 mois (IC à 95 % : 14.5, 20.0) (HR 0.64 ; IC à 95 % : 0.49, 0.84 ; p=0.001).
Dans cet essai, les personnes ayant reçu Enhertu avaient le plus souvent des nausées, de la fatigue, de l'alopécie, des vomissements, de l'anémie, de la constipation, une diminution de l'appétit, de la diarrhée et des douleurs musculo-squelettiques. Un avertissement encadré alertant les professionnels de la santé de la possibilité de lésions embryo-fœtales et de maladie pulmonaire interstitielle est inclus dans les informations de prescription.
Les patientes atteintes d'un cancer du sein doivent recevoir 5.4 mg/kg d'Enhertu en perfusion intraveineuse une fois toutes les trois semaines (sur un cycle de 21 jours) jusqu'à ce que la maladie progresse ou qu'il y ait une toxicité inacceptable.
