Le 28 novembre, la FDA a approuvé le premier biosimilaire de rituximab (Rituxan, rituximab), Truxima (rituximab-abbs, Celltrion Inc.) pour le lymphome non hodgkinien (LNH).
Rituximab is a monoclonal antibody against CD20. It is widely used in non-Hodgkin’s lymphoma and can be used in combination with chemotherapy or alone.
Le médicament d'origine était le rituxan de Roche (rituximab), qui a été approuvé pour la première fois aux États-Unis en 1997. Il existe d'autres indications pour ce produit, y compris le traitement de la polyarthrite rhumatoïde.
Le nouveau biosimilaire est Truxima (Rituximab-abbs) de Celltrion. Plus précisément, il s'applique aux patients adultes:
1) En rechute ou réfractaire, de bas grade ou follicule, LNH à lymphocytes B CD20 positifs en monothérapie
2) En tant que follicule non traité auparavant, LNH à lymphocytes B CD20 positifs associés à une chimiothérapie de première intention et patients ayant obtenu une réponse complète ou partielle au rituximab associé à une chimiothérapie, en monothérapie
3) En tant que chimiothérapie de première intention à base de cyclophosphamide, vincristine et prednisone (CVP), non progressive (y compris une maladie stable), de bas grade, CD20 positif, LNH à cellules B en tant que médicament unique
The precautions for this biosimilar are the same as the original drug, including the risk of infusion reactions, severe skin and oral reactions (some with fatal consequences); hepatitis B virus reactivation and progressive multifocal leukoencephalopathy The FDA noted that the most common side effects are infusion reactions, fever, lymphopenia, chills, infection, and weakness. It is recommended that healthcare providers monitor patients for tumor lysis syndrome, adverse cardiac reactions, nephrotoxicity, intestinal obstruction, and perforation. Patients should not be vaccinated during treatment.
