2022 juin : Après deux ou plusieurs lignes de traitement systémique, la FDA a accordé à tisagenlecleucel (Kymriah, Novartis Pharmaceuticals Corporation) une approbation accélérée pour les patients adultes atteints de lymphome folliculaire (LF) récidivant ou réfractaire.
The approval was based on the results of the ELARA trial (NCT03568461), a multicenter, single-arm, open-label trial evaluating tisagenlecleucel, a CD19-directed chimeric antigen receptor (CAR) T cell therapy, in adult patients who were refractory or relapsed within 6 months after completing two or more lines of systemic therapy (including an anti-CD20 antibody and an alkylating agent) or Tisagenlecleucel was given as a single intravenous infusion after lymphodepleting chemotherapy, with a target dosage of 0.6 to 6.0 x 108 CAR-positive viable T cells.
Le taux de réponse global (ORR) et la durée de réponse (DOR), tels qu'établis par un comité d'examen indépendant, étaient les principales mesures d'efficacité. Le TRG était de 86 % (IC à 95 % : 77, 92) parmi 90 patients dans l'analyse d'efficacité principale, avec un taux de RC de 68 % (IC à 95 % : 57, 77). Le DOR médian n'a pas été atteint, 75 % des répondants (IC à 95 % : 63, 84) répondant toujours après 9 mois. Le TRG était de 86 % (IC à 95 % : 77, 92) pour tous les patients ayant eu une leucaphérèse (n = 98), avec un taux de RC de 67 % (IC à 95 % : 57, 76).
Cytokine syndrome de libération, infection, weariness, musculoskeletal pain, headache, and diarrhoea were the most prevalent adverse effects in patients (>20 percent). 0.6 to 6.0 x 108 CAR-positive viable T cells is the suggested tisagenlecleucel dose.
