Am 28. November genehmigte die FDA das erste Rituximab-Biosimilar (Rituxan, Rituximab), Truxima (Rituximab-abbs, Celltrion Inc.), zur Behandlung des Non-Hodgkin-Lymphoms (NHL).
Rituximab is a monoclonal antibody against CD20. It is widely used in non-Hodgkin’s lymphoma and can be used in combination with chemotherapy or alone.
Das ursprüngliche Medikament war Rituxan (Rituximab) von Roche, das erstmals 1997 in den USA zugelassen wurde. Es gibt andere Indikationen für dieses Produkt, einschließlich der Behandlung von rheumatoider Arthritis.
Das neue Biosimilar ist Truxima (Rituximab-abbs) von Celltrion. Konkret gilt für erwachsene Patienten:
1) Als rezidivierendes oder refraktäres, niedriggradiges oder follikelhaftes CD20-positives B-Zell-NHL als Monotherapie
2) Als zuvor unbehandelter Follikel, CD20-positiv, B-Zell-NHL in Kombination mit Erstlinien-Chemotherapie und Patienten, die eine vollständige oder teilweise Reaktion auf Rituximab in Kombination mit Chemotherapie als Einzelwirkstoff-Erhaltungstherapie erreichten
3) Als Erstlinien-Chemotherapie mit Cyclophosphamid, Vincristin und Prednison (CVP), nicht fortschreitend (einschließlich stabiler Erkrankung), niedriggradig, CD20-positiv, B-Zell-NHL als Einzelmedikament
The precautions for this biosimilar are the same as the original drug, including the risk of infusion reactions, severe skin and oral reactions (some with fatal consequences); hepatitis B virus reactivation and progressive multifocal leukoencephalopathy The FDA noted that the most common side effects are infusion reactions, fever, lymphopenia, chills, infection, and weakness. It is recommended that healthcare providers monitor patients for tumor lysis syndrome, adverse cardiac reactions, nephrotoxicity, intestinal obstruction, and perforation. Patients should not be vaccinated during treatment.
