Noong Nobyembre 28, inaprubahan ng FDA ang unang rituximab (Rituxan, rituximab) biosimilar, Truxima (rituximab-abbs, Celltrion Inc.) para sa non-Hodgkin's lymphoma (NHL).
Ang Rituximab ay isang monoclonal antibody laban sa CD20. Malawakang ginagamit ito sa lymphoma na hindi Hodgkin at maaaring magamit kasabay ng chemotherapy o nag-iisa.
Ang orihinal na gamot ay ang Rituxan (rituximab) ni Roche, na unang naaprubahan sa Estados Unidos noong 1997. Mayroong iba pang mga pahiwatig para sa produktong ito, kabilang ang paggamot ng rheumatoid arthritis.
Ang bagong biosimilar ay Truxima (Rituximab-abbs) mula sa Celltrion. Partikular, nalalapat ito sa mga pasyente na may sapat na gulang:
1) Bilang relapsed o matigas ang ulo, mababang grado o follicle, CD20 positibong B cell NHL bilang monotherapy
2) Bilang isang dating hindi ginagamot na follicle, positibo ang CD20, B-cell NHL na sinamahan ng first-line chemotherapy, at mga pasyente na nakamit ang kumpleto o bahagyang tugon sa rituximab na sinamahan ng chemotherapy, bilang isang paggamot sa pagpapanatili ng solong-ahente
3) Bilang first-line cyclophosphamide, vincristine at prednisone (CVP) na chemotherapy, hindi progresibo (kabilang ang matatag na sakit), mababang antas, positibo sa CD20, B cell NHL bilang isang solong gamot
The precautions for this biosimilar are the same as the original drug, including the risk of infusion reactions, severe skin and oral reactions (some with fatal consequences); hepatitis B virus reactivation and progressive multifocal leukoencephalopathy The FDA noted that the most common side effects are infusion reactions, fever, lymphopenia, chills, infection, and weakness. It is recommended that healthcare providers monitor patients for tumor lysis syndrome, adverse cardiac reactions, nephrotoxicity, intestinal obstruction, and perforation. Patients should not be vaccinated during treatment.
