Em fevereirory 2023, the Food and Drug Administration (FDA) sped up the approval of tucatinib (Tukysa, Seagen Inc.) and trastuzumab for the treatment of RAS wild-type HER2-positive colorectal cancer that has spread or can not be removed after fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy.
An open-label, multicenter experiment called MOUNTAINEER (NCT03043313) examined effectiveness in 84 patients. Patients needed to have previously received treatment with fluoropyrimidine, oxaliplatin, irinotecan, and an anti-vascular endothelial growth factor (VEGF) monoclonal antibody in addition to having HER2-positive, RAS wild-type, unresectable, or metastatic colorectal cancer (mAb). People who needed an anti-programmed cell death protein-1 mAb also had cancers that did not have mismatch repair (dMMR) proteins or had a lot of microsatellite instability (MSI-H). Patients who had previously received anti-HER2 targeted therapy were not eligible.
Os pacientes receberam tucatinibe 300 mg por via oral duas vezes ao dia junto com trastuzumabe (ou um produto de trastuzumabe não licenciado para uso nos Estados Unidos) administrado em uma dose de ataque de 8 mg/kg por via intravenosa no Dia 1 do Ciclo 1 e uma dose de manutenção de 6 mg/kg kg no dia 1 de cada ciclo subsequente de 21 dias. Os pacientes receberam tratamento até o aparecimento de efeitos colaterais inaceitáveis.
A taxa de resposta geral (ORR) e a duração da resposta (DOR), conforme determinado por uma revisão central independente cega, foram as principais medidas de eficácia (RECIST versão 1.1.). O DOR mediano foi de 12.4 meses (95% CI: 8.5, 20.5) e o ORR foi de 38% (95% CI: 28, 49).
Diarréia, letargia, erupção cutânea, náusea, desconforto abdominal, respostas relacionadas à infusão e pirexia foram os efeitos colaterais mais frequentes (20%). Aumento da creatinina, hiperglicemia, ALT, diminuição da hemoglobina, AST, bilirrubina, aumento da fosfatase alcalina, diminuição dos linfócitos, diminuição da albumina, diminuição dos leucócitos e diminuição do sódio foram as anormalidades laboratoriais mais prevalentes (20%).
Em conjunto com trastuzumabe, uma dose de 300 mg de tucatinibe por via oral duas vezes ao dia é recomendada até que a doença progrida ou haja toxicidade inaceitável.
