Op 28 november keurde de FDA de eerste rituximab (Rituxan, rituximab) biosimilar, Truxima (rituximab-abbs, Celltrion Inc.) voor non-Hodgkin-lymfoom (NHL) goed.
Rituximab is a monoclonal antibody against CD20. It is widely used in non-Hodgkin’s lymphoma and can be used in combination with chemotherapy or alone.
Het oorspronkelijke medicijn was Roche's Rituxan (rituximab), dat voor het eerst werd goedgekeurd in de Verenigde Staten in 1997. Er zijn andere indicaties voor dit product, waaronder de behandeling van reumatoïde artritis.
De nieuwe biosimilar is Truxima (Rituximab-abbs) van Celltrion. Het is met name van toepassing op volwassen patiënten:
1) Als recidiverend of refractair, laaggradig of follikel, CD20-positieve B-cel NHL als monotherapie
2) Als een niet eerder behandelde follikel, CD20-positief, B-cel NHL gecombineerd met eerstelijns chemotherapie, en patiënten die een volledige of gedeeltelijke respons bereikten op rituximab in combinatie met chemotherapie, als onderhoudsbehandeling met één middel
3) Als eerstelijns chemotherapie met cyclofosfamide, vincristine en prednison (CVP), niet-progressieve (inclusief stabiele ziekte), laaggradige, CD20-positieve, B-cel NHL als enkelvoudig geneesmiddel
The precautions for this biosimilar are the same as the original drug, including the risk of infusion reactions, severe skin and oral reactions (some with fatal consequences); hepatitis B virus reactivation and progressive multifocal leukoencephalopathy The FDA noted that the most common side effects are infusion reactions, fever, lymphopenia, chills, infection, and weakness. It is recommended that healthcare providers monitor patients for tumor lysis syndrome, adverse cardiac reactions, nephrotoxicity, intestinal obstruction, and perforation. Patients should not be vaccinated during treatment.
