En Févrierry 2023, the Food and Drug Administration (FDA) sped up the approval of tucatinib (Tukysa, Seagen Inc.) and trastuzumab for the treatment of RAS wild-type HER2-positive colorectal cancer that has spread or can not be removed after fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy.
An open-label, multicenter experiment called MOUNTAINEER (NCT03043313) examined effectiveness in 84 patients. Patients needed to have previously received treatment with fluoropyrimidine, oxaliplatin, irinotecan, and an anti-vascular endothelial growth factor (VEGF) monoclonal antibody in addition to having HER2-positive, RAS wild-type, unresectable, or metastatic colorectal cancer (mAb). People who needed an anti-programmed cell death protein-1 mAb also had cancers that did not have mismatch repair (dMMR) proteins or had a lot of microsatellite instability (MSI-H). Patients who had previously received anti-HER2 targeted therapy were not eligible.
Les patients ont reçu du tucatinib 300 mg par voie orale deux fois par jour avec du trastuzumab (ou un produit de trastuzumab non homologué aux États-Unis) administré à une dose de charge de 8 mg/kg par voie intraveineuse le jour 1 du cycle 1 et une dose d'entretien de 6 mg/ kg le jour 1 de chaque cycle de 21 jours suivant. Les patients ont reçu un traitement jusqu'à l'apparition d'effets secondaires inacceptables.
Le taux de réponse globale (ORR) et la durée de la réponse (DOR), tels que déterminés par un examen central indépendant en aveugle, étaient les principales mesures d'efficacité (RECIST version 1.1.). Le DOR médian était de 12.4 mois (IC à 95 % : 8.5, 20.5) et l'ORR était de 38 % (IC à 95 % : 28, 49).
La diarrhée, la léthargie, les éruptions cutanées, les nausées, l'inconfort abdominal, les réponses liées à la perfusion et la pyrexie étaient les effets secondaires les plus fréquents (20 %). L'augmentation de la créatinine, l'hyperglycémie, l'ALT, la diminution de l'hémoglobine, de l'AST, de la bilirubine, l'augmentation de la phosphatase alcaline, la diminution des lymphocytes, la diminution de l'albumine, la diminution des leucocytes et la diminution du sodium étaient les anomalies biologiques les plus fréquentes (20 %).
En association avec le trastuzumab, une dose de 300 mg de tucatinib par voie orale deux fois par jour est conseillée jusqu'à progression de la maladie ou en cas de toxicité inacceptable.
