V únorury 2023, the Food and Drug Administration (FDA) sped up the approval of tucatinib (Tukysa, Seagen Inc.) and trastuzumab for the treatment of RAS wild-type HER2-positive colorectal cancer that has spread or can not be removed after fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy.
An open-label, multicenter experiment called MOUNTAINEER (NCT03043313) examined effectiveness in 84 patients. Patients needed to have previously received treatment with fluoropyrimidine, oxaliplatin, irinotecan, and an anti-vascular endothelial growth factor (VEGF) monoclonal antibody in addition to having HER2-positive, RAS wild-type, unresectable, or metastatic colorectal cancer (mAb). People who needed an anti-programmed cell death protein-1 mAb also had cancers that did not have mismatch repair (dMMR) proteins or had a lot of microsatellite instability (MSI-H). Patients who had previously received anti-HER2 targeted therapy were not eligible.
Pacienti dostávali tucatinib 300 mg perorálně dvakrát denně spolu s trastuzumabem (nebo produktem s trastuzumabem, který není registrován pro použití ve Spojených státech amerických) podávaným v úvodní dávce 8 mg/kg intravenózně v den 1 cyklu 1 a udržovací dávkou 6 mg/ kg v 1. den každého následujícího 21denního cyklu. Pacienti byli léčeni až do nástupu nepřijatelných vedlejších účinků.
Celková četnost odpovědi (ORR) a trvání odpovědi (DOR), jak bylo stanoveno zaslepeným nezávislým centrálním hodnocením, byly klíčovými měřítky účinnosti (RECIST verze 1.1.). Medián DOR byl 12.4 měsíce (95% CI: 8.5, 20.5) a ORR byl 38 % (95% CI: 28, 49).
Nejčastějšími nežádoucími účinky (20 %) byly průjem, letargie, vyrážka, nauzea, břišní diskomfort, reakce související s infuzí a horečka. Zvýšený kreatinin, hyperglykémie, ALT, snížený hemoglobin, AST, bilirubin, zvýšená alkalická fosfatáza, snížené lymfocyty, snížený albumin, snížený počet leukocytů a snížený sodík byly nejčastějšími laboratorními abnormalitami (20 %).
Ve spojení s trastuzumabem se doporučuje dávka 300 mg tucatinibu perorálně dvakrát denně, dokud onemocnění neprogreduje nebo dokud nedojde k nepřijatelné toxicitě.
